Your father asked what day it was, and when you told him, he asked again twenty minutes later. Your parents&#8217; domestic help mentions, almost in passing, that &#8220;Madam has been forgetting to take her tablets some days.&#8221; None of it feels like an emergency. All of it feels like something you should probably keep an eye on.<\/p>\n<p>The hard part is knowing which kind of forgetfulness this is. Some memory change is a completely normal part of ageing. Some is the earliest visible sign of something that needs a doctor&#8217;s attention \u2014 and the earlier it&#8217;s looked at, the more options there are.<\/p>\n<h2 id=\"normal-vs-warning\">Where normal ageing forgetfulness ends<\/h2>\n<p>Forgetting where you kept the car keys is normal at any age. Forgetting what the keys are for is not. That distinction \u2014 between occasionally misplacing information and losing the framework to make sense of it \u2014 is roughly where normal age-related forgetfulness ends and something worth evaluating begins.<\/p>\n<p>A few reference points that are usually reassuring on their own:<\/p>\n<ul>\n<li>Occasionally forgetting a name, then remembering it later<\/li>\n<li>Needing to write things down more than before<\/li>\n<li>Taking slightly longer to find the right word in conversation<\/li>\n<li>Losing track of the date and then correcting it once reminded<\/li>\n<\/ul>\n<p>None of these, by themselves, point to a medical problem. They become worth a closer look when they start to interfere with things your parent has always managed independently \u2014 paying bills, following a recipe they&#8217;ve cooked for decades, recognising a regular visitor, or finding their way somewhere familiar.<\/p>\n<div class=\"key-stat\">\n<div class=\"key-stat-num\">8.8M<\/div>\n<div class=\"key-stat-label\">Indians over 60 are estimated to be living with dementia, according to a large national study \u2014 and roughly 9 in 10 of them are never formally diagnosed. Underdiagnosis, not overdiagnosis, is the actual problem in India.<\/div>\n<\/p><\/div>\n<h2 id=\"warning-signs\">Signs that go beyond normal ageing<\/h2>\n<p>These are the patterns that warrant a proper evaluation \u2014 not because any one of them confirms a diagnosis, but because they represent a change from how your parent has always functioned, not a variation within it.<\/p>\n<div class=\"symptom-grid\">\n<div class=\"symptom-card\">\n<div class=\"symptom-card-title\">Repeating the same question<\/div>\n<div class=\"symptom-card-body\">Asking the same question multiple times within one conversation, with no memory of having already asked and been answered.<\/div>\n<\/p><\/div>\n<div class=\"symptom-card\">\n<div class=\"symptom-card-title\">Getting lost somewhere familiar<\/div>\n<div class=\"symptom-card-body\">Confusion about how to get to a place they&#8217;ve been going to for years \u2014 the market, a relative&#8217;s house, their own neighbourhood.<\/div>\n<\/p><\/div>\n<div class=\"symptom-card\">\n<div class=\"symptom-card-title\">Difficulty with familiar tasks<\/div>\n<div class=\"symptom-card-body\">Struggling to follow a recipe, manage bank transactions, or operate appliances they&#8217;ve used competently for decades.<\/div>\n<\/p><\/div>\n<div class=\"symptom-card\">\n<div class=\"symptom-card-title\">Withdrawal or personality change<\/div>\n<div class=\"symptom-card-body\">Pulling back from social activities, hobbies, or conversation they used to enjoy, or a noticeable shift in mood or temperament.<\/div>\n<\/p><\/div>\n<div class=\"symptom-card\">\n<div class=\"symptom-card-title\">Poor judgement in decisions<\/div>\n<div class=\"symptom-card-body\">Unusual financial decisions, falling for scams they would previously have recognised, or lapses in personal hygiene.<\/div>\n<\/p><\/div>\n<div class=\"symptom-card\">\n<div class=\"symptom-card-title\">Misplacing items in odd places<\/div>\n<div class=\"symptom-card-body\">Not just losing things, but putting them somewhere illogical \u2014 keys in the fridge \u2014 and being unable to retrace their steps to find them.<\/div>\n<\/p><\/div>\n<\/p><\/div>\n<h2 id=\"why-families-miss-it\">Why families in India often miss this \u2014 or dismiss it<\/h2>\n<p>Three things tend to delay families from acting, and all three are common enough to name directly.<\/p>\n<p><strong>Distance.<\/strong> A phone call once a week catches tone of voice, not day-to-day function. Repeating a story is easy to notice on a call; forgetting to eat lunch or take medication is not. This is one of the most consistent gaps we see with NRI families in particular \u2014 the parent sounds fine on the phone and is not fine at home.<\/p>\n<p><strong>Attribution to age.<\/strong> &#8220;She&#8217;s 78, of course she forgets things&#8221; is a reasonable-sounding explanation that is very often wrong. Age is a risk factor for cognitive decline, not an explanation for it. Treating every change as inevitable ageing means real, evaluable, sometimes treatable causes go unexamined.<\/p>\n<p><strong>Stigma and denial.<\/strong> Both the parent and the family may resist raising it \u2014 the parent out of fear or embarrassment, the family out of not wanting to believe it. This delay has a real cost: some causes of memory loss are far more treatable early than late.<\/p>\n<h2 id=\"causes\">Not all memory loss is dementia \u2014 and that distinction matters<\/h2>\n<p>This is the part that gets skipped most often in casual conversation about &#8220;memory problems,&#8221; and it matters clinically. A meaningful proportion of memory and confusion symptoms in older adults are caused by something other than a progressive neurological condition \u2014 and many of these causes are reversible with treatment.<\/p>\n<ul>\n<li><strong>Thyroid dysfunction<\/strong> \u2014 an underactive thyroid can cause slowed thinking and memory problems that resolve with correct treatment<\/li>\n<li><strong>Vitamin B12 deficiency<\/strong> \u2014 common in older adults and can present with confusion or memory changes<\/li>\n<li><strong>Medication side effects<\/strong> \u2014 sedatives, certain antihistamines, and other common drugs can cause confusion in older patients, especially in combination (see our article on <a href=\"\/blog\/polypharmacy-elderly-kolkata\/\">polypharmacy in elderly patients<\/a> for more on this)<\/li>\n<li><strong>Untreated depression<\/strong> \u2014 can present as memory and concentration problems that are sometimes mistaken for dementia<\/li>\n<li><strong>Infection-related delirium<\/strong> \u2014 a urinary tract infection or other infection can cause sudden confusion in an elderly person, distinct from gradual cognitive decline<\/li>\n<li><strong>Poor sleep or sensory decline<\/strong> \u2014 untreated hearing or vision loss can look like confusion when it is actually a person not fully perceiving what&#8217;s around them<\/li>\n<\/ul>\n<div class=\"callout callout-info\">\n<div class=\"callout-title\">Why this distinction matters<\/div>\n<p>If memory changes are being caused by a thyroid problem, a vitamin deficiency, a medication interaction, or an infection, treating the underlying cause can substantially improve or even resolve the symptoms. If the changes are due to a progressive condition, early diagnosis still matters \u2014 for planning, for safety, and for treatments that work better the earlier they start. Either way, the only way to know which situation you&#8217;re in is a proper clinical evaluation, not a guess based on age.<\/p>\n<\/p><\/div>\n<h2 id=\"nri-distance\">If you&#8217;re noticing this from abroad<\/h2>\n<p>For families managing a parent&#8217;s care from outside India, the practical question isn&#8217;t &#8220;is this dementia&#8221; \u2014 it&#8217;s &#8220;how do I get an honest picture of what&#8217;s actually happening, and get it evaluated, without being there.&#8221;<\/p>\n<ul>\n<li><strong>Ask specific questions, not general ones.<\/strong> &#8220;How are you doing?&#8221; invites a reassuring answer. &#8220;What did you have for lunch today?&#8221; or &#8220;Did you take your evening tablets?&#8221; gets you something you can actually assess.<\/li>\n<li><strong>Loop in whoever is physically present<\/strong> \u2014 a caretaker, a neighbour, a sibling in the city \u2014 and ask them directly whether they&#8217;ve noticed repetition, confusion, or missed medications. Families are often surprised that household help has noticed changes weeks before anyone mentioned it to them.<\/li>\n<li><strong>Don&#8217;t wait for a crisis to arrange an evaluation.<\/strong> A home visit from a geriatric specialist can be arranged and coordinated remotely \u2014 you don&#8217;t need to be physically present in Kolkata to get your parent properly assessed.<\/li>\n<\/ul>\n<h2 id=\"what-a-specialist-does\">What a geriatric specialist evaluation actually looks at<\/h2>\n<p>A structured evaluation for memory concerns is broader than a quick conversation. It typically includes a detailed history from both the patient and a family member or caregiver who sees them day-to-day, a review of all current medications for anything that could be contributing, blood tests to rule out thyroid or vitamin-related causes, and a structured assessment of memory, thinking, and everyday function \u2014 comparing how your parent is managing now against how they managed before.<\/p>\n<p>The goal of this evaluation isn&#8217;t to hand a family a label. It&#8217;s to work out what&#8217;s actually driving the change, treat what&#8217;s treatable, and \u2014 where the cause is a progressive condition \u2014 put a plan in place early, while there&#8217;s the most room to act on it.<\/p>\n<p>If you&#8217;ve noticed a change in your parent that doesn&#8217;t feel like ordinary forgetfulness, that instinct is worth acting on.
